| BCMPA Registration Form 2008-2009Please send this with $20.00 for your annual registration. Please send to Rick Maynard for the Fraser Valley or Dee MacDonald for Maple Ridge and also include your filled out Waiver and Code of Conduct forms .
Novice: 11 or under as of December 31, 2008 Junior: 14 and under as of December 31, 2008 Senior: 20 and under as of December 31, 2008 Open: Any athlete over 21 as of December 31, 2008 or who is competing internationally Masters: 35 or over as of December 31, 2008 Publicity Agreement: I hereby give permission to the BC Modern Pentathlon Association and the member clubs to use the name or photograph of the undersigned member for the BCMPA website, in club newsletters and in articles or reports of activities used on the radio or in newspapers, magazines or other media which may be used by BCMPA for publicity or communications. If you do not agree with the Publicity Agreement, please clearly cross out it's entire section above. Waiver: To be signed by all members and parent when athlete is under 19 years of age. Disclaimer Clause: Canadian Modern Pentathlon, British Columbia Section is not responsible for any loss or damage suffered by any person(s), by the use of air pistols, fencing equipment or participation in equestrian events, running or swimming for the purpose of training, competitions or clinics: or for any loss or damage suffered by any person(s) travelling to and from their place of residence or competition. Agreement: I agree to assume all risks involved in the use of air pistols, fencing equipment and participation in equestrian , running or swimming events for the purpose of training, clinics or competitions: or for any loss or damage suffered by a person(s) travelling to and from their place of residence or competition with myself and agree to pay the cost of any emergency treatment that may become necessary. I agree to hold harmless Canadian Modern Pentathlon Association, British Columbia Section, its agents and servants relieving themselves of all liability for losses or damages of all and every description.
